Immunizations and Vaccines

Every child residing in Indiana who is enrolled in an accredited school shall be immunized as determined by the state department of health against: diphtheria, pertussis (whooping cough), tetanus, measles, rubella, poliomyelitis, mumps, varicella, hepatitis A, hepatitis B, and meningitis per Indiana Code 20-34-4-2. Please make sure your student is up-to-date as additional vaccines are required prior to students entering Kindergarten, 6th, and 12th grade.

If your student cannot get vaccinated per state requirements, an exemption form must be completed and submitted to the nurse at your students building. If your child has a medical reason to not get vaccinated, we require your doctor to complete the medical exemption form. If you don't want your child to get the required vaccinations based on religious preferences, please complete a religious exemption form. Please note in the event of an outbreak of a vaccine preventable disease for which your child is not fully vaccinated, your child may be excluded from school to protect his/her health and the health of all our students and staff.
